MUST HAVE APPROVED APPLICATION WITH AWARDS CHAIR TO PURCHASE

• Certificates and medallions are included in the application fee. 
  • These must be ordered within the “Awards” calendar year they are earned. (August 11^th to August 10^th)
• Previous Award recipients can purchase medallion at reduced cost.
• Plaques may be purchased for an additional cost. 
  • These must be ordered within the “Awards” calendar year they are earned. (August 11^th to August 10^th)
